    Consultation/Equipment Set-Up/Training: $78/hour

Travel Costs for Evaluations and Training 

  Within 35 mile radius (On-Site) Over 35 miles Over 225 miles
Evaluations $0 .25 per mile + $30/hr travel time .25 per mile + $60/hr travel time
Equipment Set-up / Training $0 .25 per mile + $30/hr travel time .25 per mile + $60/hr travel time
Per Diem and Hotel (as per government rate) Not Applicable $26 + $40 respectively/ 24 hrs $26 + $40 respectively/ 24 hrs

Note about application of travel fees: Travel fees will only be implemented once per trip.  In other words, if more than one individual is set up to be evaluated or to receive training at one location, the travel fees are only applied once.
